OBJECTIVE To describe two consecutive spontaneous pregnancies during luteal-phase down-regulation with a GnRH-a. METHODS Case report. METHODS University-affiliated reproductive medicine unit. METHODS A 33-year-old woman with a history of failed uterine tubal reanastomosis. METHODS Four IVF cycles. METHODS Pregnancy. RESULTS The patient had two spontaneous pregnancies during luteal-phase down-regulation. CONCLUSIONS Although midluteal administration of GnRH-a is not established as a treatment, there might be a beneficial effect on the endometrium via the medication's evoked gonadotropin flare.

UI MeSH Term Description Entries
D007987 Gonadotropin-Releasing Hormone A decapeptide that stimulates the synthesis and secretion of both pituitary gonadotropins, LUTEINIZING HORMONE and FOLLICLE STIMULATING HORMONE. GnRH is produced by neurons in the septum PREOPTIC AREA of the HYPOTHALAMUS and released into the pituitary portal blood, leading to stimulation of GONADOTROPHS in the ANTERIOR PITUITARY GLAND. D008183 Luteal Phase The period in the MENSTRUAL CYCLE that follows OVULATION, characterized by the development of CORPUS LUTEUM, increase in PROGESTERONE production by the OVARY and secretion by the glandular epithelium of the ENDOMETRIUM. The luteal phase begins with ovulation and ends with the onset of MENSTRUATION.
