Conservative management and vaginal delivery following ruptured renal angiomyolipoma. 2001

BACKGROUND Rupture of a renal angiomyolipoma during pregnancy is rare and occasionally fatal. Usually emergency nephrectomy is required, with cesarean simultaneously in patients at 28 weeks' gestation or later. We have been unable to find reports of cases in which conservative treatment for a ruptured angiomyolipoma was followed by successful vaginal delivery. METHODS A woman experienced retroperitoneal hemorrhage resulting from angiomyolipoma rupture at 27 weeks. She was managed conservatively and subsequently delivered vaginally after induction at 38 weeks. The tumor was treated by embolization postpartum. CONCLUSIONS Successful vaginal delivery in a pregnant woman with a ruptured angiomyolipoma is possible under strict monitoring of the maternal and fetal status.
