[Special problems of pacemaker-therapy in infants and children (author's transl)]. 1976

A Schwingshackl, and H Schwingshackl

The special problems of pacemaker-therapy in infants and children are reported and compared with those of adults. The possibilities of application of pacemakers and the indications for temporary or permanent use are discussed. Special attention is paid to the disadvantages of presently available pacemakers for children due to electro-physiologic and hemodynamic facts. Complications of pacemaker-therapy and wishes in regard to technical improvements of pacemakers are discussed too. The possibilities of checking pacemaker patients are also mentioned. The therapeutic value of pacemaker for pediatric patients is analysed, using the results of the literature and an own experience with an infant, which is discussed in detail.

D010138 Pacemaker, Artificial A device designed to stimulate, by electric impulses, contraction of the heart muscles. It may be temporary (external) or permanent (internal or internal-external). D011247 Pregnancy The status during which female mammals carry their developing young (EMBRYOS or FETUSES) in utero before birth, beginning from FERTILIZATION to BIRTH. Gestation,Pregnancies
D001919 Bradycardia Cardiac arrhythmias that are characterized by excessively slow HEART RATE, usually below 50 beats per minute in human adults. They can be classified broadly into SINOATRIAL NODE dysfunction and ATRIOVENTRICULAR BLOCK. Bradyarrhythmia,Bradyarrhythmias,Bradycardias
D005260 Female Females
D006327 Heart Block Impaired conduction of cardiac impulse that can occur anywhere along the conduction pathway, such as between the SINOATRIAL NODE and the right atrium (SA block) or between atria and ventricles (AV block). Heart blocks can be classified by the duration, frequency, or completeness of conduction block. Reversibility depends on the degree of structural or functional defects. D006332 Cardiomegaly Enlargement of the HEART, usually indicated by a cardiothoracic ratio above 0.50. Heart enlargement may involve the right, the left, or both HEART VENTRICLES or HEART ATRIA. Cardiomegaly is a nonspecific symptom seen in patients with chronic systolic heart failure (HEART FAILURE) or several forms of CARDIOMYOPATHIES. D006333 Heart Failure A heterogeneous condition in which the heart is unable to pump out sufficient blood to meet the metabolic need of the body. Heart failure can be caused by structural defects, functional abnormalities (VENTRICULAR DYSFUNCTION), or a sudden overload beyond its capacity. Chronic heart failure is more common than acute heart failure which results from sudden insult to cardiac function, such as MYOCARDIAL INFARCTION.
