Continence after radical prostatectomy is a multifactorial problem with no clear common denominator. At the heart of success is proper patient selection and operative skill in the precise dissection of the sphincteric anatomy. In general, surgeons who know the anatomy protect the patient by virtue of less blood loss, better margins of resection, and greater functional preservation.
