A new "extended" myocutaneous flap has been designed by fasciocutaneous extension anteriorly and inferiorly from the 6th to 8th slips of the serratus anterior muscle. This flap has been used as a pedicled flap for various types of head and neck reconstructions and satisfactory results obtained. This article describes the anatomy of the flap, surgical technique and typical cases.
