OBJECTIVE To describe a technique for suturing a dislocated intraocular lens (IOL) with 25-gauge (G) instruments. METHODS Non-comparative interventional case series performed at a single center. Five patients with a dislocated IOL underwent 25-G transconjunctival sutureless vitrectomy (25G-TSV) with a temporary externalization of the haptics to fixate the IOL. Three IOLs enclosed in the lens capsule were dislocated into the vitreous and removed from the capsule with 25G-TSV using perfluorocarbon liquid. The best-corrected visual acuity and surgical complications resulting from this technique were recorded. RESULTS In all cases, the IOL was fixed stably and remained well positioned. The visual acuity was improved or maintained in all cases. There were no intraoperative or postoperative complications except for one eye with a postoperative iris capture of the sutured IOL. CONCLUSIONS Findings indicate that 25G-TSV is a practical and safe method for fixating a dislocated IOL.
