Subcutaneous dissection of fluid and toxicity from the chosen antimicrobial agent may occur when using a closed suction-irrigation system. Continuous rather than intermittent suction, and frequent examination of the treated extremity should minimize the risk of subcutaneous fluid dissection. Nephrotoxic agents are dangerous in closed suctionirrigation and should be avoided whenever possible.
