A case of borderline-lepromatous leprosy is presented to highlight the detection and prevalence of florid, new cases of leprosy towards the lepromatous end of the spectrum in the northern and western regions of Australia; the recognition and early treatment of lepra reactions; and the potential use of an enzyme-linked immunosorbent assay to detect and to monitor immunoglobulin M levels in leprosy.
