Acute retrobulbar neuritis complicating herpes zoster ophthalmicus. 1987

Acute retrobulbar neuritis occurred 24 days after an episode of Herpes zoster ophthalmicus in an otherwise healthy, 19-year-old man. Profound visual loss with consecutive optic atrophy ensued. Neurologic evaluation was normal and supported the presumptive diagnosis of parainfectious Herpes zoster retrobulbar neuritis.

Clinical features include retro-orbital pain that is aggravated by eye movement, loss of color vision, and contrast sensitivity that may progress to severe visual loss, an afferent pupillary defect (Marcus-Gunn pupil), and in some instances optic disc hyperemia and swelling.
