Described are the cases of three acute cardiac patients and their referral to the emergency department by two health maintenance organization triage systems. The HMOs' methods did not use existing emergency medical services systems, and by policy virtually discouraged free use of the emergency medical services system. The patients required hospitalization and extensive ED treatment.
