A 34-year-old female had longstanding hypogammaglobulinemia associated with extensive nodular lymphoid hyperplasia (NLH) of the small bowel, colon and gallbladder. The characteristic appearance in double contrast barium studies should alert the radiologist to the possibility of hypogammaglobulinemia.
