Oral manifestations of the chronic graft-v-host reaction. 1983

B Rodu, and J P Gockerman

The graft-v-host reaction (GVHR), a major complication of bone marrow transplantation, is a complex immunologic phenomenon with a diverse and often confounding presentation. After reviewing major systemic manifestations, this report details the clinical and pathological oral findings of chronic GVHR. The reaction is characterized clinically by a mucosal lichenoid keratosis. Pathological findings resemble mucosal lichen planus, and minor salivary gland show lesions similar to those of Sjögren's syndrome. These oral findings are potentially valuable in the early recognition and follow-up of chronic GVHR. The differential diagnosis includes viral infections and chemotherapeutic drug toxicity, but these can usually be eliminated from consideration by viral cultures and temporal correlation of symptoms, respectively.

D007972 Leukoplakia, Oral A white patch seen on the oral mucosa. It is considered a premalignant condition and is often tobacco-induced. When evidence of Epstein-Barr virus is present, the condition is called hairy leukoplakia (LEUKOPLAKIA, HAIRY). D009061 Mouth Mucosa Lining of the ORAL CAVITY, including mucosa on the GUMS; the PALATE; the LIP; the CHEEK; floor of the mouth; and other structures. The mucosa is generally a nonkeratinized stratified squamous EPITHELIUM covering muscle, bone, or glands but can show varying degree of keratinization at specific locations. D011183 Postoperative Complications Pathologic processes that affect patients after a surgical procedure. They may or may not be related to the disease for which the surgery was done, and they may or may not be direct results of the surgery. Complication, Postoperative,Complications, Postoperative,Postoperative Complication
D002908 Chronic Disease Diseases which have one or more of the following characteristics: they are permanent, leave residual disability, are caused by nonreversible pathological alteration, require special training of the patient for rehabilitation, or may be expected to require a long period of supervision, observation, or care (Dictionary of Health Services Management, 2d ed). For epidemiological studies chronic disease often includes HEART DISEASES; STROKE; CANCER; and diabetes (DIABETES MELLITUS, TYPE 2). D006087 Graft vs Host Reaction An immunological attack mounted by a graft against the host because of HISTOINCOMPATIBILITY when immunologically competent cells are transplanted to an immunologically incompetent host; the resulting clinical picture is that of GRAFT VS HOST DISEASE.
