Correlation of real-time ultrasonic placental grading with amniotic fluid lecithin/sphingomyelin ratio. 1983

K M Tabsh

Placental grading with real-time ultrasound was performed on 235 patients who underwent amniocentesis for determination of the lecithin/sphingomyelin (L/S) ratio between 31 and 44 weeks' gestation. Grades I, II, and III placentas were encountered in 88, 72, and 75 patients, respectively. Forty-eight percent of Grade I, 69% of Grade II, and 92% of Grade III placentas had mature L/S ratios (L/S greater than or equal to 2.0). All of the 52 patients with a Grade III placenta who underwent repeat cesarean section at term gestation had mature L/S ratios. However, 37% of the pregnancies complicated by maternal or fetal disease, with Grade III placentas, had immature L/S ratios. The gestational age of these six patients ranged from 34 to 37 weeks. These results suggest that a Grade III placenta in normal term gestation has a good correlation with fetal pulmonary maturity as predicted by the amniotic fluid L/S ratio.

UI MeSH Term Description Entries
D008168 Lung Either of the pair of organs occupying the cavity of the thorax that effect the aeration of the blood. Lungs
D010713 Phosphatidylcholines Derivatives of PHOSPHATIDIC ACIDS in which the phosphoric acid is bound in ester linkage to a CHOLINE moiety. D010920 Placenta A highly vascularized mammalian fetal-maternal organ and major site of transport of oxygen, nutrients, and fetal waste products. It includes a fetal portion (CHORIONIC VILLI) derived from TROPHOBLASTS and a maternal portion (DECIDUA) derived from the uterine ENDOMETRIUM. The placenta produces an array of steroid, protein and peptide hormones (PLACENTAL HORMONES). Placentoma, Normal,Placentome,Placentas,Placentomes
D011247 Pregnancy The status during which female mammals carry their developing young (EMBRYOS or FETUSES) in utero before birth, beginning from FERTILIZATION to BIRTH. Gestation,Pregnancies
D005260 Female Females
D005325 Fetal Organ Maturity Functional competence of specific organs or body systems of the FETUS in utero. Fetal Maturity, Functional,Functional Fetal Maturity,Maturity, Fetal Organ,Maturity, Functional Fetal,Organ Maturity, Fetal
D005865 Gestational Age The age of the conceptus, beginning from the time of FERTILIZATION. In clinical obstetrics, the gestational age is often estimated from the onset of the last MENSTRUATION which is about 2 weeks before OVULATION and fertilization. It is also estimated to begin from fertilization, estrus, coitus, or artificial insemination. D006801 Humans Members of the species Homo sapiens. Homo sapiens,Man (Taxonomy),Human,Man, Modern,Modern Man
D000649 Amniocentesis Percutaneous transabdominal puncture of the uterus during pregnancy to obtain amniotic fluid. It is commonly used for fetal karyotype determination in order to diagnose abnormal fetal conditions. Amniocenteses
D000653 Amniotic Fluid A clear, yellowish liquid that envelopes the FETUS inside the sac of AMNION. In the first trimester, it is likely a transudate of maternal or fetal plasma. In the second trimester, amniotic fluid derives primarily from fetal lung and kidney. Cells or substances in this fluid can be removed for prenatal diagnostic tests (AMNIOCENTESIS).
