The superior mesenteric artery and vein can easily be approached through a subduodenal left retroperitoneal route and dissected. The procedure proved extremely useful in right hemicolectomy, cephalic duodeno-pancreatectomy, mesentero-caval anastomosis and direct interventions on the superior mesenteric artery.
