[Anaesthesia in the extremely obese patient (author's transl)]. 1978

G Kreienbühl

This paper reviews the literature on anaesthetic problems in extremely obese patients. The main concerns of the anaesthetist in these patients is pulmonary function, especially hypoxaemia. The pathogenesis of hypoxia is explained by an increased tendency to airway closure by the weight of fat, which compresses the thorax. The significance of the patient's position during and after operation is stressed. The Pickwick-Syndrome is extremely rare, even in large series of obese patients, but its diagnosis is important for therapeutic consequences. For anaesthesia in these patients neuroleptanaesthesia as well as classical inhalational procedures can be used.

UI MeSH Term Description Entries
D008176 Lung Volume Measurements Measurement of the amount of air that the lungs may contain at various points in the respiratory cycle. Lung Capacities,Lung Volumes,Capacity, Lung,Lung Capacity,Lung Volume,Lung Volume Measurement,Measurement, Lung Volume,Volume, Lung
D009765 Obesity A status with BODY WEIGHT that is grossly above the recommended standards, usually due to accumulation of excess FATS in the body. The standards may vary with age, sex, genetic or cultural background. In the BODY MASS INDEX, a BMI greater than 30.0 kg/m2 is considered obese, and a BMI greater than 40.0 kg/m2 is considered morbidly obese (MORBID OBESITY).
D010347 Patient Care Planning Usually a written medical and nursing care program designed for a particular patient. Nursing Care Plans,Goals of Care,Plans, Nursing Care,Care Goal,Care Goals,Care Plan, Nursing,Care Planning, Patient,Care Plans, Nursing,Nursing Care Plan,Plan, Nursing Care,Planning, Patient Care
D010845 Obesity Hypoventilation Syndrome HYPOVENTILATION syndrome in very obese persons with excessive ADIPOSE TISSUE around the ABDOMEN and DIAPHRAGM. It is characterized by diminished to absent ventilatory chemoresponsiveness; chronic HYPOXIA; HYPERCAPNIA; POLYCYTHEMIA; and long periods of sleep during day and night (HYPERSOMNOLENCE). It is a condition often related to OBSTRUCTIVE SLEEP APNEA but can occur separately. Pickwickian Syndrome,Obesity-Hypoventilation Syndrome,Hypoventilation Syndrome, Obesity,Obesity-Hypoventilation Syndromes
D011187 Posture The position or physical attitude of the body. Postures
D012119 Respiration The act of breathing with the LUNGS, consisting of INHALATION, or the taking into the lungs of the ambient air, and of EXHALATION, or the expelling of the modified air which contains more CARBON DIOXIDE than the air taken in (Blakiston's Gould Medical Dictionary, 4th ed.). This does not include tissue respiration ( Breathing
D003011 Closing Volume The lung volume at which the dependent lung zones cease to ventilate presumably as a result of airway closure. Closing Volumes,Volume, Closing,Volumes, Closing
D006801 Humans Members of the species Homo sapiens. Homo sapiens,Man (Taxonomy),Human,Man, Modern,Modern Man
D000273 Adipose Tissue Specialized connective tissue composed of fat cells (ADIPOCYTES). It is the site of stored FATS, usually in the form of TRIGLYCERIDES. In mammals, there are two types of adipose tissue, the WHITE FAT and the BROWN FAT. Their relative distributions vary in different species with most adipose tissue being white. D000402 Airway Obstruction Any hindrance to the passage of air into and out of the lungs. Choking,Airway Obstructions,Obstruction, Airway,Obstructions, Airway
