Forty-five cases of atlanto axial dislocations were seen during a period of 12 years. Forty of them were congenital dislocations. A little over one-third of them were under 12 years of age. Neurological signs are said to be uncommon in children but exaggerated jerks were found in every case. Sixteen of the cases were irreducible, three of them under 12 years. Thirteen transoral anterior decompressions and 31 posterior fusions were done in this group. There were seven deaths, two before surgery, three following anterior surgery and two after posterior surgery. Except for two, all the rest showed good recovery. Our experience with emphasis on diagnosis in children and a few technical aspects of surgery are presented.
