I. Chirurgische Abteilung, Allgemeines Krankenhaus Barmbek, Hamburg.

The preperitoneal approach with implantation of a dacron sheet was applied in 37 patients with recurrent hernias and in 30 patients with primary bilateral hernias. In total 120 hernias were repaired. The patients were controlled prospectively for serious complications. The patients were examined every 3 months in the first and every 6 months in the second year after operation. The mean follow-up time was 17.2 months. RESULTS In one patient with a combined hernia an indirect hernia sac was not detected intraoperatively, so it persisted. Recurrences were not observed. COMPLICATIONS (hematoma, fluid collections, wound infections, intraabdominal adhesions) occurred in 13.2 of the patients. CONCLUSIONS The preperitoneal implantation of a dacron sheet is a safe and reliable procedure for treatment of recurrent groin hernias and primary hernias with a high risk of recurrence. The rate of infection is low under preventing measures.

D008875 Middle Aged An adult aged 45 - 64 years. Middle Age
D010537 Peritoneum A membrane of squamous EPITHELIAL CELLS, the mesothelial cells, covered by apical MICROVILLI that allow rapid absorption of fluid and particles in the PERITONEAL CAVITY. The peritoneum is divided into parietal and visceral components. The parietal peritoneum covers the inside of the ABDOMINAL WALL. The visceral peritoneum covers the intraperitoneal organs. The double-layered peritoneum forms the MESENTERY that suspends these organs from the abdominal wall. D006552 Hernia, Inguinal An abdominal hernia with an external bulge in the GROIN region. It can be classified by the location of herniation. Indirect inguinal hernias occur through the internal inguinal ring. Direct inguinal hernias occur through defects in the ABDOMINAL WALL (transversalis fascia) in Hesselbach's triangle. The former type is commonly seen in children and young adults; the latter in adults.
