In severe trauma dead tissue, pain, stress, hemorrhage and ischaemia-reperfusion injury put an 'antigenic load' on defense systems which may result in 'host defense failure disease' and multiple systems organ failure. Surgical management must eliminate or reduce 'antigenic load' as soon as possible. Besides shock treatment, control of hemorrhage and repair of hollow viscus injuries, unstable 'central' fractures must be fixed, dead tissue removed and compressed muscle compartments released. Fracture fixation in multiple trauma requires modifications concerning monitoring, positioning and operative methods.
