With the repeated regional application of 20 mg rt-PA distally with a deep venous thrombosis, successful fibrinolysis is possible. Best results have been achieved in single location thrombosis. Extended thrombotic occlusions of calf, popliteal, thigh and pelvic veins could be recanalized in a lower percentage. The basic precondition is a compression of the subcutaneous veins of the extremity with bandages to minimize the collateral blood flow and to force the fibrinolytic agent near the thrombosis. Bleeding complications and other side effects in the 162 patients treated up to now are lower compared with systemic application of fibrinolytic therapy.
