The original technique of in plus high tibial osteotomy with the use of CZ-2 (after Czyrny) external distractor/compressor in proximal tibia varus deformity is presented. It has been used in 5 cases with 3 years follow-up. Fixing the device to the bone before performing an incomplete osteotomy allows for precise correction and prevents intraoperative complications. If desired correction exceeds 10 degrees, two stages are recommended: first intraoperative one with filling of the created defect with frozen bone graft and final, postoperative one, performed according to bone lengthening principles.
