Modern total knee arthroplasty is a successful surgical procedure for the vast majority of patients. The success of knee replacement has extended its indications, and this has created controversy. Surgical principles for total knee arthroplasty include: establishment of proper mechanical alignment, preservation of joint line, maintenance of joint stability, maintaining extensor mechanism function and rigid fixation of components. Continued research and evaluation of long-term arthroplasty results will aid in answering current controversies surrounding knee replacement.
