BACKGROUND Both a persistent primitive trigeminal artery (PTA) and the absence of an internal carotid artery are rare developmental anomalies. RESULTS Two cases with the unusual association of a persistent PTA and absence of the internal carotid artery on the left side are reported. The left anterior cerebral artery territory was supplied from the right internal carotid artery via the anterior communicating artery, and the left middle cerebral artery territory was supplied from the vertebrobasilar system via the persistent PTA antidromically. CONCLUSIONS This is the first report of such an association of developmental anomalies of cerebral vessels. Our cases are instructive for gaining insight into the involution and evolution processes of primitive cerebral arteries in the human embryo.

D014276 Trigeminal Nerve The 5th and largest cranial nerve. The trigeminal nerve is a mixed motor and sensory nerve. The larger sensory part forms the ophthalmic, mandibular, and maxillary nerves which carry afferents sensitive to external or internal stimuli from the skin, muscles, and joints of the face and mouth and from the teeth. Most of these fibers originate from cells of the TRIGEMINAL GANGLION and project to the TRIGEMINAL NUCLEUS of the brain stem. The smaller motor part arises from the brain stem trigeminal motor nucleus and innervates the muscles of mastication.
